Summary

Resident #32, who had hypothyroidism, had a provider order dated 01/08/26 for levothyroxine 137 mcg by mouth once daily and lab work in two months to check TSH. On 02/22/26, nursing staff documented that when they went to give the resident the morning levothyroxine, they found the dose on the medication card in the med cart was incorrect. The note stated the dose had been changed on 01/08/26, the medication was not given that morning, and staff were unsure how long the incorrect dose had been administered because the medication cards were dated 01/21/26 and 02/05/26. The dose given that day was documented as 125 mcg. On 02/23/26, the physician was notified and ordered levothyroxine 137 mcg PO daily to continue, canceled the TSH for 03/04/26, and moved the TSH to mid-April 2026. The medical record did not contain documentation of a TSH level in mid-April as ordered. During interview on 06/11/26, an administrative staff member confirmed the facility failed to collect the TSH level as ordered after the medication error was identified. The record states Resident #32 received 45 doses of levothyroxine at the wrong dose before staff realized the error.
