Failure to Document and Provide Scheduled Shower and Grooming Care

Summary

The facility failed to provide documented evidence that showers were provided to a resident who was scheduled to receive showers every Wednesday and Saturday. For Resident 228, who was admitted with diagnoses including malignant neoplasm of the colon and difficulty walking, the point-of-care task charting showed scheduled shower/bathe care, but the record contained only one shower sheet for April and May 2026. The electronic charting also showed only two showers documented in April, while the remaining shower days were marked as NA or RR. During interviews, a CNA, LPN, ADON, and DON explained that if a resident refused a shower, staff were to encourage the resident, notify the nurse, complete a shower sheet, and document the refusal and attempts made. The ADON and DON stated shower sheets were expected for each shower day, whether the shower was given or refused, and that the sheets were also used to monitor skin assessments. The DON acknowledged the NA documentation did not reflect the care provided or whether the resident refused, and that the lack of shower sheets or inconsistent EHR entries reflected that the resident was not provided any shower attempts. The facility policy titled Shower/Tub Bath stated that if a resident refused a shower or tub bath, staff were to notify the licensed nurse and document the reasons and interventions taken. The report also included findings for another resident with impaired cognition and dependence for personal hygiene, whose fingernails were thickened and long, and for a cognitively intact resident who was dependent on staff for grooming and had fingernails extending more than 0.25 inch beyond the fingertips with dark matter beneath multiple nails. Staff acknowledged the nails were excessively long and should have been trimmed and cleaned, but the residents’ nail care had not been provided as expected.
