Summary

Pharmaceutical services were not provided in accordance with professional standards of practice when an LPN preparing medications for a resident dropped a Bisacodyl tablet onto the medication cart and discarded it into a trash receptacle attached to the cart instead of the designated medication disposal container. During the same medication pass, the resident was given medications without the nurse identifying the medications being administered. The nurse also completed a controlled substance count with the outgoing nurse but did not sign the shift verification on the Controlled Drug - Key Exchange Audit form as expected. Controlled substance handling was also inaccurate for another resident. An LPN stated she had administered one tablet of Norco to the resident but had not yet documented the removal or administration on the Controlled Drug Record form. Review of the resident’s blister pack and controlled drug record showed a mismatch in the remaining tablet count and no documentation of the dose given at the time it was administered. During the same observation, the nurse left the medication cart unlocked while she went to inspect medications stored in the refrigerator, and later acknowledged the cart should remain locked when not under direct supervision. Record review showed repeated administration of routine and scheduled medications outside the facility’s established time frame for multiple residents, with no documentation that physicians were notified of the delays. Residents with diagnoses including multiple sclerosis, diabetes, pressure ulcers, cardiomyopathy, COPD, dysphagia, heart failure, and other chronic conditions had medications scheduled for morning, noon, and afternoon times that were often given one to several hours late, including doses administered in the late morning, early afternoon, and even late afternoon. The facility’s audit review confirmed these medications were expected to be given within one hour before or after the scheduled time, and the records showed no evidence of physician notification for the repeated delayed administrations.
