Summary

The facility failed to provide safe discharge teaching and caregiver training, including return demonstration and evaluation of the caregiver’s ability to assist with ADLs, for a resident discharged to a hotel with her son identified as the primary caregiver. The resident was admitted with diagnoses including DM, CVA with left-sided weakness, difficulty walking, and need for assistance with personal care. Her MDS indicated she had no cognitive deficits, but she required dependent assistance with showering/bathing and substantial or partial assistance with lower body dressing, footwear, personal hygiene, tub/shower transfers, walking 10 feet, toilet transfers, chair/bed transfers, and sit-to-lying. Social services documentation showed the resident’s son requested discharge planning and stated he wanted his mother to return in his care. The resident stated she wanted to discharge to her son’s home temporarily, and she became tearful and said she wanted to be with her son. The SSA stated she knew the son stayed at hotels intermittently and did not believe he had a permanent home. The resident was discharged to a hotel, and the SSA stated there was no documentation that APS was notified despite concerns about the son’s unstable housing and the discharge destination. Therapy and nursing interviews confirmed the resident still required assistance with toileting, transfers, ambulation, and mobility and was not independently ambulatory. The DOR stated therapy did not re-evaluate the resident for discharge readiness and did not complete caregiver training with the son before discharge. The LN stated she provided diabetic and medication education to the resident on the day of discharge, but there was no return demonstration for self-administration and no physician order for self-administration training. Because the son was not present, no caregiver teaching or competency evaluation was completed with him. The DON stated the discharge plan should have been updated with the IDT and acknowledged that caregiver training and return demonstrations were necessary for safe discharge, while also stating the son’s behavior had raised safety concerns. The facility record contained no documented APS notification, and the facility policy required evaluation of discharge needs, caregiver availability, capacity, and capability to perform required care.
