Call Light Not Within Reach for Resident With Visual Impairment
Summary
The facility failed to provide reasonable accommodation of a resident’s needs and preferences when Resident #38’s call light was not within reach on two separate observations. Resident #38 was a male admitted on 02/10/2026 with diagnoses including schizoaffective disorder, bipolar type, blindness in both eyes, type 2 diabetes, and depression. His quarterly MDS showed a BIMS score of 00, indicating severe cognitive impairment. His care plan, revised 03/31/2026, stated that the call bell would be attached to a stuffed animal so he could feel for it and ring for help when needed. During an observation and interview on 03/31/2026, Resident #38 was in bed awake, and his call light was observed hanging on the wall and clamped together, not within his reach. When asked, he said he had to reach to get the light and was observed struggling to sit up. During a second observation and interview on 04/02/2026, the call light was still hanging on the wall and not within reach. Resident #38 stated he could not reach it and told the surveyor to ask staff how to press the light. Staff interviews confirmed the call light placement had not been adapted for the resident’s needs. A CNA stated call lights should be within reach, but explained the resident was blind and the cord was on the wall because he frequently twisted and turned, creating a risk of entanglement. The CNA also stated she had not reviewed the care plan and had not seen a stuffed animal attached to the current system. An LVN stated she was unaware why the call light was hanging on the wall and said she would need to review the care plan. The MDS Coordinator acknowledged concern that the call light should be within reach and stated the new call light system required an adaptive attachment that had not yet been implemented. The DON stated staff had been doing routine rounds and the resident was monitored with hourly checks, while the Administrator stated call lights should always be within reach and accessible.
