Summary

The facility failed to provide a safe, clean, comfortable, and homelike environment for 2 of 19 residents reviewed for resident rights when room temperatures were not maintained within the expected range. On 03/31/2026, Resident #56, an older male with diabetes mellitus, hypertension, and macular degeneration, was observed in bed and stated his room was cold. He was covered with multiple blankets. At 9:28 AM and again at 9:30 AM, his room temperature was measured at 68 degrees and then 67 degrees using a PDT650 thermometer. Resident #59, an older male with hypertension, diabetes mellitus, and major depressive disorder, was also observed in bed on 03/31/2026 and stated his room was cold while covered with multiple blankets. At 9:32 AM, the air conditioning control box in the hall outside the two residents’ rooms was observed set at 68 degrees, and the plastic cover for the control box was missing. The DOSS stated at 10:00 AM that the control box should have had a cover and that he would look into it. Later that day, the DOSS stated the facility temperatures should be maintained between 72 and 82 degrees and that staff must have changed it. The Administrator later stated he expected temperatures to be maintained between 71 and 81 degrees to ensure resident comfort.
