The DON stated she was aware that NA C and NA F had both been employed for greater than four months without completion of the required competency program, and explained that NA C had not taken the test due to personal issues. She said NA F had just reached four months of employment and had not taken her test yet due to issues with FBI fingerprinting, a name change after marriage, and credentialing from a nursing program completed outside the country. The DON also stated there was no facility policy related to competency program completion by nursing assistants.

The DON stated LVN D was responsible for making sure nurse aides were certified, and the ADMN stated her expectation was that nurse aides complete training and the CNA test within 180 days of working full-time; if the clinical test was failed, they should be moved to hospitality aid until they passed. The facility policy required nurse aides to demonstrate competency in skills and techniques necessary to care for residents' needs, and the job description stated nurse aides must be certified or complete state-required training and competency evaluation within four months of employment.

The DON stated awareness that several NAs were working but was not aware that some were past the four‑month limit, and reported having no involvement with Human Resources or knowledge of the terminations and rehires related to lack of certification.
