Infection Control Failures With PPE Use, EBP Communication, and Glucometer Cleaning

Summary

The facility failed to maintain an effective infection prevention and control program when staff did not use the required PPE during care of a resident with a PICC line who was on enhanced barrier precautions (EBP). Resident 106 had diagnoses including neutropenia and a stage 4 pressure ulcer, and the record showed an active order for a PICC line. During medication administration, an LN was observed giving IV antibiotic through the PICC line while wearing gloves only and not a gown, even though EBP signage outside the room indicated that gloves and a gown were required for device care or use of a central line. The LN later confirmed that gown use was omitted and stated both gloves and gown should have been worn when using and caring for the PICC line. The facility also did not consistently communicate which residents were on EBP. Resident 106 was identified by the IP as being on EBP, but the resident was not included in the care plan, and the IP stated the facility did not obtain a physician order for residents on EBP. For Resident 163, the record showed diagnoses including diabetes mellitus, recurrent MRSA bacteremia, and a venous ulcer, but there was no order for EBP. Staff were unsure whether Resident 163 was on EBP, the IP confirmed the resident was on EBP, and the IP also confirmed that no EBP sign was posted outside the room. The IP stated that without the sign, staff may not know the resident is on EBP and may fail to gown and glove. The facility also had glucometers that were not clean. During observation, one glucometer had a red smear on it, which an LN disinfected and the DSD stated was most likely blood and not cleaned properly. At another nursing station, two glucometers had blue smears on the top, and an LN verified the smears were present and stated they were control solution. The IP stated staff are supposed to clean glucometers after every use and after control solution is used, and the facility policy stated glucometers are to be cleaned and disinfected after each use and according to manufacturer instructions for multi-resident use.
