Unsafe Food Handling and Equipment Sanitation Practices
Summary
Food was not consistently prepared, stored, served, or distributed in accordance with professional food safety standards in the facility’s kitchen and skilled nursing facility dietary areas. During observation and interview, surveyors found both ice machines with visible residue and buildup inside the evaporator areas. In the main kitchen ice machine, the stationary engineer found orange-red slimy residue on the bottom of the evaporator unit, and the chief engineer confirmed the equipment was not clean. At the skilled nursing facility ice machine, black slimy residue and orange buildup were observed on the evaporator, and the food-nutrition services manager confirmed it was not clean. The outside technician later identified red/pink marks at the evaporator in the main kitchen machine as biofilms. Surveyors also observed multiple food-contact surfaces and storage practices that did not meet standards. Four of five cooking pans had black buildup and deep scratches, and the food-nutrition services manager stated the pans were not smooth and should be replaced. In the walk-in refrigerator, opened or prepared foods including diced carrots, gravy, tortilla rolls, and cheese were found without labels and dates. In dry storage, three opened bags of pasta were also found without open dates or use-by dates. The food-nutrition services manager confirmed the items should have been labeled with open and use-by dates and noted the facility used a label printer for that purpose. Additional observations showed a can opener blade with chipped metal coating and wear, and the food-nutrition services manager stated it needed replacement. Food thawing in the walk-in refrigerators was not marked with the date pulled from the freezer or the use-by/discard date, including multiple pans and boxes of cooked and raw meats, soups, and other items. Two cooks were observed wearing masks as beard restraints while their facial hair remained exposed on both sides. One nutrition assistant could not correctly describe the manual dishwashing process for the 3-compartment sink, initially giving incorrect water temperature and sanitizer immersion time before changing the answer after reviewing the posted instructions. The food-nutrition services manager and director of food and nutrition services acknowledged the thawing, facial hair restraint, and dishwashing practices were not being followed correctly.
