Failure to Account for Resident Belongings After Hospital Transfer

Trabuco Hills Post AcuteLake Forest, California Survey Completed on 05-27-2026

Summary

The facility failed to protect one resident’s belongings from theft or loss after the resident was transferred to an acute care hospital. The facility’s policy stated that residents’ personal items would be inventoried at admission, added to the inventory if brought in later, protected from theft or loss, and returned to the resident’s representative after discharge. In this case, the resident was transferred to the hospital on 5/11/26, and the family later reported that the resident’s cellphone, eyeglasses, and wallet with a driver’s license were not sent with the resident. Review of the resident’s Clothing and Possessions form showed the clothing, cellphone, and eyeglasses were listed on admission, but the wallet with the driver’s license was not documented. The discharge section of that form was blank. The eINTERACT Transfer Form showed a section for personal belongings sent with the resident, but none of the boxes were checked. The resident’s family told the facility they would not be returning and asked about the belongings, but facility staff were unable to locate the items when the family came to pick them up. The SSD and DON stated they were trying to locate the missing belongings and that the facility would reimburse items listed on the inventory.

D
F0557 F557: Honor the resident's right to be treated with respect and dignity and to retain and use personal possessions.
Short Summary

A resident with an indwelling suprapubic catheter was observed in bed with an uncovered urinary drainage bag lying on the floor beside the bed and visible from the hallway. The resident’s care plan called for a dignity bag to cover the urine bag each shift and for the catheter bag and tubing to be positioned below the bladder and away from the room entrance, but the bag remained uncovered during repeated observations. An LPN confirmed the bag was visible and could not explain why staff did not cover it after the resident returned from a recent urology visit with a different drainage bag.
