The care plan described verbal behavioral symptoms and directed staff to observe for changes in behavior, document, and report to the doctor, but the surveyor could not find behavioral charting in the electronic record. During interviews, the DON stated the facility did not have behavioral/effectiveness monitoring, and the NHA also stated there was no behavioral monitoring for this resident.
