The CFO stated the facility did not learn the file was rejected until a letter was received in the mail from CMS on 04/22/2026, and that a later attempt to resubmit the data was too late for Quarter 4 reporting.

During interview, the Administrator stated that the issue may have been due to incorrect transcription from the timeclock reading and that the Business Office Associate needed to compare findings with the DON’s daily staffing sheets before reporting. The Administrator later stated the report was not correct because it did not accurately reflect the actual hours worked and that staffing information should be reported accurately.
