Summary

The facility failed to prevent physical abuse for 1 of 8 residents reviewed for abuse investigations. During a resident-to-resident incident, Resident 2 initiated physical aggression toward Resident 1 by grabbing Resident 1’s left wrist, which caused unwanted physical contact and resulted in swelling and bruising. The facility’s abuse policy stated that residents have the right to be free from abuse and defined abuse as the willful infliction of injury, including acts that are deliberate even if injury was not intended. An incident summary documented that nursing staff witnessed Resident 2 agitated and grabbing Resident 1’s left wrist, causing a superficial bruise. Resident 1 stated that she asked Resident 2 to move so they could pass in the hallway, after which Resident 2 became upset and grabbed and pulled her left wrist. Resident 1 later described the wrist as swollen, black and blue, and painful at first. Skin observations showed no bruising four days before the incident and a fading bruise four days after the incident. Staff interviews confirmed training on abuse and acknowledged that resident-to-resident physical contact can constitute physical abuse; the Administrator and DON also stated that Resident 1 sustained a physical injury as a direct result of the incident.
