Summary

The facility failed to provide a safe, clean, sanitary, comfortable, and homelike environment for 3 of 7 resident rooms and 1 of 2 hallways reviewed for environment, including Room #2, Room #4, Room [ROOM NUMBER], Room [ROOM NUMBER], and Hallway #1. Observations found Room #2's air conditioning unit had gaps to the outside, Room #4 was missing baseboards below the air conditioning unit, Room [ROOM NUMBER] had a stained floor under the air conditioning unit, and Room [ROOM NUMBER] had a white casing board partially attached to the wall and patch work below the air conditioning unit that was uneven and in poor repair. Hallway #1 on the secure men's unit had a broken window seal exposing wood fragments. During interview, the Maintenance Director stated the air conditioning unit in Room [ROOM NUMBER] had started leaking and that mud plaster had been placed over it, and he said there should not be space in the air conditioning unit to the outside. He also stated the whiteboard was a racetrack used to hide cables and phone lines and described it as an eye sore. He said he was working on getting a new window for the men's secured unit because the window seal was broken and exposed wood. The DCO and ADM both stated the room and unit aesthetics were unpleasant and that the facility expected resident rooms to be homelike. The facility policy titled, Quality of Life-Homelike Environment, stated residents were to be provided with a safe, clean, comfortable, and homelike environment.
