Summary

The facility failed to establish and maintain an infection prevention and control program for a resident with a tracheostomy, indwelling catheter, and open skin areas who was on enhanced barrier precautions (EBP). The resident’s record showed diagnoses including chronic respiratory failure, obesity, quadriplegia, bipolar disorder, neuromuscular dysfunction of the bladder, and tracheostomy status. Her care plan identified EBP related to the foley catheter, tracheostomy, and open skin areas, and noted she was at risk for infection and pain. The care plan also documented that she scraped areas of skin and attempted to color them with pens and markers. During an observation, the ADON performed a skin assessment and incontinent care for the resident without washing her hands before putting on gloves and without applying PPE. The ADON cleansed the resident’s skin, reapplied barrier ointment, changed the resident’s brief, and did not use hand sanitizer between glove changes during the assessment. The resident had red areas under the creases of her left breast and between her legs. A container with PPE was in the room, and a yellow sign above the bed indicated EBP. During the same encounter, a CNA entered the room to assist with positioning and holding the resident. The CNA washed her hands and applied gloves only, but did not apply PPE before assisting with care. In interviews, the CNA stated she believed PPE was only needed for coughing, feces odor, bad wounds, or staph infection, and said she did not have to wear PPE while providing care to the resident. The ADON stated she should have worn PPE and should have stopped to put it on. Other staff interviews showed inconsistent understanding of EBP, with some staff stating they only wore gloves, some saying they did not know what EBP meant, and others identifying different residents or conditions as requiring PPE. The facility policy and CDC guidance reviewed stated that EBP requires gown and glove use during high-contact care activities for residents with wounds or indwelling medical devices, including urinary catheters and tracheostomies.
