Summary

The facility failed to ensure medications were transcribed accurately into a resident’s electronic medical record and failed to identify a high-risk medication ordered for a resident with end-stage renal disease. For one sampled resident receiving dialysis, nursing management entered admission orders from hospital discharge paperwork, but the medication administration record showed carvedilol 6.25 mg once daily and clonidine 0.2 mg once daily, while the hospital discharge orders called for carvedilol 6.25 mg twice daily and clonidine 0.2 mg twice daily. A nursing progress note documented that the carvedilol order was outside the recommended dose or frequency because the daily frequency was below the usual twice-daily frequency. The same resident had end-stage renal disease, dialyzed Tuesday/Thursday/Saturday, and had a pattern of refusing dialysis treatments. A provider later ordered baclofen 5 mg twice daily for chronic back pain, and the record noted prior ineffective treatments with multiple other pain medications. Facility staff did not take into context the high-risk nature of baclofen in a resident with renal failure and dialysis dependence. Staff member B stated the staff trusted provider orders and did not know nurses would question baclofen for a resident on dialysis. After the resident became lethargic and was noted to be initially not arousable, with critical potassium notification and concern from the family about slow breathing, the provider documented that baclofen was stopped and Dilaudid was decreased. The resident was transferred to the hospital emergency room for family/patient request with elevated blood pressure and oxygen saturation of 92%. The hospital later documented encephalopathy with highest suspicion for baclofen toxicity and recommended discontinuing and never restarting the medication.
