Unsanitary and Damaged Kitchen Food Service Equipment

Summary

The facility failed to maintain food service equipment and kitchen surfaces in a sanitary condition during the recertification survey kitchen tour. On 03/04/2026 at 10:10 AM, surveyors toured the kitchen with the Certified Dietary Manager and observed multiple unsanitary and damaged conditions, including an unclean ice machine, a cracked and damaged overflow cover on the prep sink, a broken drain stopper that prevented the vats from being filled with water, a leaking prep-sink drain line, and rusted, unclean floor sink covers. Surveyors also observed a prep table supporting the coffee machine with multiple jagged holes, a coffee machine drain line without a backflow prevention device, a hand sink not properly sealed to the wall, nonfunctional ceiling-mounted heating furnaces, and kitchen electrical panel doors left open with dust, debris, and dirt accumulation. Additional findings included warped and water-damaged laminated wooden panels around a prep table, unclean walls and floors beneath cooking equipment, torn caulking around a hand sink, a nonfunctional drain stopper at an in-counter prep sink, improperly fitted ceiling tiles with dust and debris, unclean compressor fan covers and door gaskets in the walk-in refrigerator, a walk-in freezer that was nonfunctional and turned off, and rusted, unclean shelving casters. In the warewashing area, the hand sink was not securely sealed to the wall, the 3-compartment sink dump test failed and greywater overflowed onto the kitchen floor, the faucets were not securely fitted, and the drain lines were manifolded. Surveyors also observed exposed wall areas behind the dish machine, uneven grouting, ventilation that failed to capture steam and vapors, unfitted ceiling tiles, a chemical bucket on the floor, leaking greywater onto a detergent drum and the kitchen floor, fruit flies beneath the pre-rinse area, and damaged utility food service carts that could not be properly cleaned and sanitized.
