Summary

The facility failed to ensure a full RN assessment was documented after a resident with dementia, repeated falls, muscle weakness, and unsteadiness on feet had an unwitnessed fall with injury. After the resident climbed out of bed independently and fell, blood was noted from the nose and the resident reported 6/10 left lower-leg pain. The post-fall note documented that the APRN and responsible party were notified, but it did not include a full assessment such as range of motion to all extremities, neurological signs, or the time the provider was notified. A later nurse's note stated the resident was assessed, assisted back to bed, and monitored per protocol, but it also did not document the full assessment findings following the fall with head injury. The facility also failed to ensure pain medication was signed out under the correct licensed nurse's name in the MAR. The resident received tramadol 25 mg for pain at 9:03 PM, but the MAR listed LPN #5 as the administering nurse even though staffing records showed LPN #5 was not working that shift. The Controlled Substance Disposition Record identified LPN #3 as the nurse who administered the tramadol. Interviews confirmed LPN #5 had left at 3:00 PM, LPN #3 administered the medication, and both nurses stated the medication should have been documented under the correct login credentials. Facility leadership stated licensed nurses should document a full assessment, including range of motion and neurological signs, after a fall with injury and should document the time of provider notification.
