Summary

The facility failed to ensure the provider was notified of changes in Resident #1’s condition after an unwitnessed fall with head injury. Resident #1 had diagnoses including dementia without behavioral disturbances, repeated falls, muscle weakness, and unsteadiness on feet. The resident’s MDS showed severe cognitive impairment, dependence for bed mobility, substantial assistance for transfers, and a history of multiple falls with injury. The care plan identified the resident as at risk for falls and required assistance with transfers, reminders to call for help, a clutter-free environment, and frequent checks. After the fall, staff documented blood from the nose, left lower-leg pain, and later left shoulder pain when the arm was raised. The APRN was notified and a left shoulder x-ray was ordered, but the record did not show that the x-ray was obtained or resulted. The next day, staff documented that the resident was alert and confused, received tramadol for shoulder pain, and was self-propelling into walls and objects. A social services note later documented increased confusion, inability to determine spatial orientation, continued bumping into walls and objects, and leaning in the wheelchair. The clinical record did not show documentation that RN #2 assessed the resident or notified the provider after the changes were identified, despite the changes beginning hours earlier. The resident was not sent to the ED until later that afternoon for increased confusion and red spots under both eyes. Hospital imaging showed a closed fracture of the proximal left humerus and a closed fracture of the nasal bone. Interviews confirmed that the resident should have been transferred immediately after the initial provider order, that the provider should have been notified when the x-ray could not be obtained and when confusion and pain continued, and that there was a delay in notifying the provider and a delay in care.
