The resident’s MDS documented that the COVID-19 vaccination was not up to date and did not include whether the vaccine had been offered or the reason it was not received. Review of the EMR did not reveal documentation that the facility offered the SARS-COV-2 vaccine to the resident or the resident’s representative.
