Failure to Provide Skin Care, Wound Monitoring, and Correct LAL Mattress Setting

Summary

The facility failed to provide necessary skin care and monitoring for a resident who was admitted with MASD to the perineum and buttocks and later returned with a pressure injury. The resident’s admission/readmission data tool showed MASD on 4/29/26, and the care plan identified risk for pressure sore development, but the medical record did not show wound care orders or documentation that the skin was monitored for the MASD. During interview, the DON confirmed there were no treatment orders or treatments rendered for the resident’s MASD before the resident was discharged to the acute hospital. After the resident returned from the hospital, the record showed a coccyx pressure injury identified by the acute care hospital as a Stage 2 pressure injury and later documented by the facility as a healing Stage 3 pressure injury and deep tissue pressure injury of the sacrum. The physician’s admission note identified a community acquired pressure-related deep tissue injury in the sacrum and a pressure ulcer of the sacrum Stage 3, but the facility record failed to show wound care orders or monitoring for the pressure injury. Staff interviews indicated the treatment nurse was expected to assess the resident’s skin the day after admission, but no additional skin assessments were found after the initial admission assessment. During observation, the resident was found with bright pink/red perineal skin with spots of broken skin, redness in the bilateral groin, and redness on the sacrum with an open spot while wearing adult briefs and lying on a low air loss mattress. The LVN stated she would wait for physician orders before providing treatment. The resident’s TAR later showed orders for treatment of perineal MASD and a sacrococcyx pressure injury. The low air loss mattress was also observed set beyond the 320-pound indicator even though staff confirmed the resident weighed 113 pounds, and staff acknowledged the mattress settings were inaccurate.
