Staff Lacked Competency to Care for Residents on Anticoagulants
Summary
The facility failed to ensure three nursing staff members demonstrated the competencies and skill sets needed to provide safe nursing care to residents receiving anticoagulant medication. Two CNAs and one LVN were interviewed and were unable to explain the care and monitoring needed for residents on blood thinners. The facility’s Certified Nurse Assistant duties included reporting changes in residents’ condition to the nurse supervisor, and the facility’s anticoagulation protocol stated staff and the physician would monitor for possible complications and manage related problems. Resident 1 was readmitted to the facility with scattered discoloration to the upper extremities, abdominal area, and right lower extremity. The resident had a physician’s order to monitor for ecchymosis, abnormal bleeding, bloody stool, coffee ground emesis, vomitus, nosebleed, and skin tears, with prompt reporting to the physician every shift related to anticoagulant use. The resident was also ordered Pradaxa 110 mg by mouth every 12 hours for atrial fibrillation, and the MAR showed multiple administrations of the medication during the review period. During interviews, CNA 3 could not explain how to care for residents at risk for bruising and bleeding, did not know what bruising was, and gave an incorrect response about what to do if bruising was noticed. CNA 2 stated residents on blood thinners should wear a gown and gloves and said to ask for help during bladder and bowel care, showering, and repositioning if they bruise and bleed easily. LVN 4 stated residents on anticoagulants were high risk for bleeding and were monitored for bleeding, but could not explain what else to monitor or where the monitoring was documented. The DSD stated CNAs knew to report changes in condition and verified there had been no in-service training provided to staff on how to care for residents on anticoagulant medications. On observation, CNA 1 was providing care and called an LVN to assess Resident 1’s skin, and bruising and discoloration were observed on the resident’s arms and under the left breast.
