Failure to Maintain Homelike Resident Rooms
Summary
The facility failed to provide a safe, clean, comfortable, and homelike environment for three sampled residents whose rooms and bathrooms were observed to have multiple areas of disrepair. Resident 2 had diagnoses including respiratory failure and congestive heart failure, and had a BIMS score of 13 of 15, indicating intact cognition. During observation of the resident’s room, surveyors found eight quarter-sized holes covered with spackle that had not been sanded or painted, a foam guard on the telephone wire with two large rips, extensive water damage behind the bathroom sink with peeling paint and no sealant, the sink separating from the wall, and a large missing chunk from the bathroom baseboard. Resident 2 stated the holes in the wall did not feel homelike and wanted them fixed. Resident 4 had diagnoses including hemiplegia and epilepsy and had a BIMS score of 15 of 15. Surveyors observed a 3 by 2 foot repaired area beneath the window near Bed C that was spackled but not sanded or painted, another 2 by 3 foot area with multiple tears in the wall from wheelchair contact that was also spackled but not painted, and a bathroom fan that sounded like loose blades moving around. In the six-bedroom unit, the dividing wall had multiple levels of damage and repair, including open broken areas, uneven spackle, and no sanding or paint. The wall under the TVs had multiple quarter-sized holes filled with spackle but not finished, and the wall by the baseboard had a 2.5 foot by 3 inch tear in the drywall with no repair or paint. Resident 4 stated he often saw spiders by the window, his sink was clogged very often, he had reported it that morning, he had no curtain or shade on the window, and the lack of these items and the clogged sink were not homelike. Resident 6 had diagnoses including cerebral infarction and encephalopathy and had a BIMS score of 15 of 15. Surveyors observed multiple repaired holes in the wall adjacent to the doorway, a 3 by 4 foot repaired area under the window, and a 2 by 2.5 foot area above an electrical outlet in the bathroom, all with spackle that was uneven, unsanded, and unpainted. Resident 6 stated she could see the holes and disrepair despite limited eyesight, felt embarrassed to have guests visit, and wanted the walls repaired. Staff interviews confirmed that damaged walls and non-functional sinks were not homelike, that repairs were expected to be timely, and that the maintenance process relied on staff writing issues in the maintenance log and following up. The Director of Maintenance stated he was not aware of several of the observed conditions, including the water damage, the loud bathroom fan, and the extent of wall disrepair, and said his monthly rounds focused mainly on safety items rather than wall disrepair.
