Infection Control Lapses With Respiratory Equipment, Hand Hygiene, and EBP Use

Summary

The facility failed to maintain an effective infection prevention and control program when Resident 54’s nebulizer mask was observed sitting uncovered on the nebulizer machine tray outside of the antimicrobial bag for an unknown period of time. Resident 54 had been admitted with COPD and acute and chronic respiratory failure, and the active physician order required Enhanced Barrier Precautions because of MDRO CRE urine culture results and an abdominal wound. During the observation, the Respiratory Therapist stated the nebulizer mask should be placed in an antimicrobial bag after each use and confirmed it should have been changed and stored that way to prevent infection. The facility also failed to follow Enhanced Barrier Precautions and hand hygiene practices during resident care. A CNA did not sanitize hands before entering Resident 54’s room and did not sanitize hands after leaving the room following direct contact. A Licensed Nurse changed Resident 54’s linens without wearing a gown, even though the resident was on EBP and linen changes were identified as a high-contact activity requiring gown and glove use. The nurse confirmed the gown should have been worn and stated that not wearing gown and gloves could transport infections to another resident. Additional observations showed similar hand hygiene lapses with other residents on EBP. A nurse had direct contact with Resident 90 and did not sanitize hands when leaving the room. A CNA did not sanitize hands after direct contact with Resident 1. Another CNA removed gown and gloves after direct contact with Resident 40 and did not sanitize hands afterward. The Infection Preventionist and DON stated staff were expected to perform hand hygiene before and after resident contact and to wear gown and gloves for residents on EBP, including during high-contact care such as changing linens.
