Record review showed a prior speech therapy progress note documented that Resident 5 had a swallow evaluation and was cleared for a regular diet with thin liquids, but initially requested to remain on pureed diet. The note also stated the resident could request a diet upgrade without a speech therapy order. The RD stated the resident had remained on pureed diet until the texture was changed on 2/17/26 and that resident preferences should be honored. The CDM confirmed the diet order was changed in the dietary system on 2/18/26 and said the one upgraded tray was a trial tray. The DON stated the diet change was not carried out when the resident requested the upgrade, and the ST stated the upgrade should have been ordered once the resident requested it because he had been cleared to upgrade or downgrade based on preference.
