Summary

The facility failed to ensure safe storage of medication for one resident. During a concurrent observation and interview on 2/17/26 at 2:36 a.m., a container of Trazadone 100 mg tablets was found in the resident’s room with directions to give 1-2 tablets at bedtime for depression. The resident’s admission record showed diagnoses including anxiety disorder, panic disorder, and depression. Review of the resident’s clinical record showed no IDT notes, no order, and no care plan indicating the resident was able to self-administer medications safely. During interviews, an LN stated that keeping medications in a resident’s possession would require a physician’s order, care plan, and IDT awareness, and that the resident’s cognitive status would be assessed. The DON stated the facility does not allow residents to keep medication in their possession unless there was a physician’s order and the IDT had assessed and approved it. The facility policy stated that medications brought in by a resident or family member are used only upon written order by the attending physician after contents are verified and if the packaging meets facility guidelines.
