Summary

The facility failed to provide adequate supervision when Resident 138 eloped from the facility without staff being aware of her whereabouts. Resident 138 was admitted with diagnoses including acute pyelonephritis, Type 1 diabetes, bacteremia, bipolar disorder, and PTSD, and she was receiving IV ertapenem to treat a bacterial infection. The record review showed that on 11/19/25, a CNA notified the charge nurse that the resident was not in her room and had last been seen about 30 minutes earlier. The medical record did not contain an individualized nursing assessment for unauthorized departure risk, documentation that her behavioral and medical history were evaluated for supervision needs, physician notification at the time of departure, verification of her safety during the absence, interdisciplinary team notes related to the elopement, or documentation of her stated intent to leave AMA or refusal to sign an AMA form. During observation, Resident 139 was in the designated smoking area with other residents and had cigarettes in her purse. Staff interviews confirmed that smoking assessments are completed on admission and that the Activities Director was responsible for them, but she was not sure whether Resident 139 was a smoker and confirmed the assessment was not done. The DON stated that every resident who smokes should have a smoking assessment completed on admission and that failure to do so could place residents at risk for safety concerns.
