For a second resident, the facility had a grievance investigation related to a fall and allegations that staff refused to send the resident to the hospital. The resident had a BIMS score of 14 and intact cognition on the quarterly MDS. The investigation documents showed APS involvement, record review, and a conclusion that the grievance was not confirmed. During interview, the administrator stated APS contacted him about the complaint, that he reviewed the record, and that he did not interview staff. He said he did not believe the allegation should have been sent to the State Agency because it could not be verified and he questioned the validity of the allegation.
