Failure to Monitor Dialysis Aftercare and Anticoagulant Side Effects
Summary
The facility failed to ensure Resident 1 was appropriately assessed, monitored, and documented after returning from dialysis treatments. Resident 1 was admitted with diagnoses including dependence on renal dialysis, chronic kidney disease, and congestive heart failure, and had provider orders for dialysis on Mondays, Wednesdays, Thursdays, and Fridays. The orders also required staff to collect the dialysis binder, complete the dialysis evaluation, and check the shunt site for pain, redness, bleeding, swelling, or non-functioning access site every hour for six hours. Review of the dialysis communication record, TAR, MAR, and dialysis binder showed staff were to complete Part A before dialysis, verify Part B was returned, and complete the dialysis evaluation when the resident returned, but there was no place for facility staff to document an assessment or other information after return. The TAR also did not provide documentation space for the required six hourly shunt checks on Thursdays, and only one box was available at 11:00 PM on Mondays, Wednesdays, and Fridays. Staff interviews confirmed the expectation was for vital signs and shunt site monitoring after dialysis, but the DNS stated there was no other dialysis communication form for additional documentation and was not sure of the specific monitoring time frame. The facility also failed to set up and monitor Resident 49 for side effects of apixaban, an anticoagulant ordered twice daily for embolism and thrombosis of the left popliteal and femoral veins. Review of the MARs and TARs for December 2025 through February 2026 showed no monitor in place to watch for and identify possible side effects of the medication. Although the anticoagulant care plan identified a risk for abnormal bleeding and included monitoring for signs such as bleeding gums, nose bleeds, or unusual bruising, staff stated the TAR should have included a monitor for bruising and bleeding to be documented every shift.
