The facility failed to ensure that a resident received routine dental services, including an annual inspection of the mouth and necessary dental care such as cleaning, fillings, or denture adjustments, as required. The resident was admitted on 04/10/25, and record review and interviews confirmed that no dental visit had occurred since admission. During an interview, the resident’s family member stated that the resident had not seen a dentist while at the facility and noted that the resident had missing teeth.
