Missing Handwashing Supplies, Improper Food Storage, and Unsanitary Food Preparation

Summary

The facility failed to ensure a handwashing station in the Aspen Pinion Serving Kitchen was stocked with disposable hand towels. During a tour on 03/30/2026 at 9:24 AM, the handwashing station was observed without disposable hand towels, and the Culinary Director confirmed the missing towels and stated they should always be available at handwashing stations. The facility policy titled, Policy and Procedure Manual - Hand Washing, dated 2021, stated handwashing facilities would be readily accessible and equipped with hot and cold running water, paper towels and/or automatic hand dryer, soap, trash cans and signage. The facility also failed to properly store dry food and to prepare and serve food in a sanitary manner. During a tour of the Reflections Satellite Pantry on 03/30/2026 at 9:45 AM, pouches of powdered fruit punch and lemonade were open and exposed to the air and other contaminants, and the Culinary Director confirmed they should have been folded and closed. During lunch tray line observation in the Quail Serving Kitchen on 04/01/2026, the Dietary Supervisor checked soft and bite sized asparagus at 132 degrees Fahrenheit and stated it was under the appropriate temperature and should be sent back to the primary kitchen to be reheated to 140 F. The Dietary Supervisor then removed a personal knife from a pocket, washed it with hand soap in the handwashing sink, and used it to cut asparagus on a resident plate. The Dietary Supervisor later stated a clean and disinfected knife should have been obtained from the pantry and confirmed the personal knife had been used to prepare resident food.
