Improper PPE Use and Incomplete Infection Surveillance

Summary

The facility failed to ensure staff used personal protective equipment properly during care of residents who were positive for COVID-19 and a resident on droplet precautions. During an observation, two staff members were in a resident’s room with enhanced droplet precautions posted on the door, but they were wearing only respirators and were not wearing isolation gowns, disposable gloves, or protective goggles. One staff member repositioned the resident while directly touching him with ungloved hands and clothing, and the resident was coughing during the care. The staff member stated she knew the resident was COVID-19 positive and had seen the precaution sign, but did not think to put on a gown and gloves. In another observation, two CNAs entered a resident’s room where droplet precautions and enhanced droplet precautions signs were posted. The resident was sitting in a wheelchair and was heard coughing frequently. The CNAs wore masks and gloves, but neither wore eye protection or an isolation gown while transferring the resident back to bed. After the care, one CNA removed her gloves and left the room without washing or sanitizing her hands, stating she would use hand sanitizer down the hallway. In a separate observation, a staff member coming out of a COVID-19 positive resident’s room wore full PPE while handling the resident’s food tray, but then removed PPE and handled other items in the hallway without washing her hands. The facility also failed to maintain a system of communicable disease surveillance for tracking active infections. Review of the January 2026 Infection Control Log showed a list of residents and antibiotics, but it did not include room numbers, admit dates, onset dates, infection sites, infection-related diagnoses, healthcare-acquired infection status, or resolution dates. The total number of infections was blank, and the log did not identify the number or types of infections. The log also contained facility maps without tracking information for infection types or resident rooms. Staff interviews indicated the infection preventionist role had recently changed and that infection surveillance logs were being updated.
