Incomplete Investigation of Resident-to-Resident Incident

Mount Ascension Transitional Care Of CascadiaHelena, Montana Survey Completed on 02-26-2026

Summary

The facility failed to complete a thorough investigation of a facility-reported incident involving resident #70, who was observed in a dining room physically interacting with a female resident, including kissing her. The incident report dated 1/26/26 showed that female residents on the wing were interviewed and the female resident involved in the incident was moved from the wing, but the event documentation did not show a complete investigation of the incident. Record review and staff interview showed the facility did not document interviews with other staff members, did not document monitoring for awareness of details or ongoing patterns of behavior for the residents involved, did not review or update resident #70's care plan, and did not include interdisciplinary team event notes regarding the incident. Staff member B stated resident #70's care plan should have been updated to include monitoring for recent changes in behavior after the physical incident with another resident, and stated that if a resident-to-resident incident occurred, monitoring or alert charting was usually started for both residents. The facility policy required the IDT to conduct a thorough review, the nurse to implement an immediate intervention and update the care plan, and the DON and/or NHA to complete final resolution of the event.

Short Summary

Incomplete Abuse Investigation: The DON investigated an allegation that a resident with dementia, depression, and a prior femur fracture was treated roughly during a shower, but only interviewed one CNA named on the abuse report. Another CNA was also assigned to the resident and documented providing the shower, yet was not interviewed, and no other residents cared for by either CNA were interviewed. The facility's abuse policy required thorough investigation, including staff and resident interviews, and the DON stated the policy was not followed.

Short Summary

Incomplete Investigation of Alleged Staff-to-Resident Sexual Abuse: The facility failed to thoroughly investigate an allegation that a CNA raped a resident during incontinence care.
