Resident #2 was repeatedly observed lying on his back with his left elbow directly on the bed, including while the elbow wound was open and undressed. Resident #71 was observed lying directly on his back without positioning pillows and stated staff did not rotate him in bed. No repositioning documentation was provided for residents #2, 71, 73, and 98, despite the facility policy requiring turning and repositioning for residents at risk of or with existing pressure injuries.
