Summary

The facility failed to ensure a resident received wound treatment and care in accordance with physician orders and professional standards for multiple wounds, including an external fixation device, a right second toe diabetic ulcer, a hematoma on the left fourth toe, right foot amputation sites, suspected deep tissue injury to the right foot toes, and right leg pin care. The resident told the surveyor that staff did not always change the dressings on the wounds on his feet, and another staff member reported that the resident had said the nurses did not consistently change the dressings on his feet and toes. Record review showed repeated treatment orders in December 2025, January 2026, and February 2026 that were not completed as ordered. These orders included daily and scheduled wound cleansing, Betadine application, skin prep, dressing changes, pin care, and quarter-turn adjustments of the external fixator by the wound nurse or DON only. The documentation showed the treatments were not completed as ordered for the resident’s right second toe, left fourth toe, right foot first toe amputation site, right foot hallux amputation site, suspected deep tissue injury to the right foot toes, and right leg pin care. A wound clinic staff member stated the resident had a complicated wound with an external fixation device and that the wound clinic sent orders and directions for wound care, but the nursing staff did not follow them. The staff member said the resident was brought to the wound clinic weekly to work around the problem with the orders not being followed. During interview, facility staff stated the wound orders were initiated and then populated on the treatment administration record, and that sign-off on the treatment record indicated completion; one staff member said it might have been her mishap when the treatment administration record was not signed off for the resident.
