Infection Control Failures During Resident Care

Summary

The facility failed to ensure staff maintained infection prevention and control practices during resident care, including hand hygiene, glove changes, and use of PPE for enhanced barrier precautions. The report states that staff did not perform hand hygiene before and after glove use as required by the facility’s hand hygiene policy, and that staff did not use gowns and gloves in rooms requiring enhanced barrier precautions during high-contact care activities. Resident 7 was admitted with diagnoses including stroke, dementia, and kidney complications. The resident’s comprehensive assessment showed moderately impaired cognition, ability to make needs known, a urinary catheter in place, and a need for wound care. During observation, Staff V and Staff T, both nursing assistants, were in the room with a nurse providing a brief change, bed bath, and wound care after putting on a gown and gloves for enhanced barrier precautions. Staff V handled soiled gloves after cleaning the resident’s first bowel movement, assisted with wound care supplies, touched a trash bag from a pocket, and touched multiple surfaces before changing gloves without hand hygiene. Staff T removed one pair of gloves with another pair underneath, continued bathing and touching drawers and clean linen with soiled gloves, and used the same soiled gloves and cloth from perineal care to perform urinary catheter care without glove change or hand hygiene. Resident 50 had diagnoses of urinary blockage and an indwelling catheter and required enhanced barrier precautions. During wound care, Staff L, an LPN, and Staff Q, a nursing assistant, washed hands and put on gloves, but Staff L changed gloves without performing hand hygiene and neither staff member wore PPE consistent with enhanced barrier precautions. Staff L stated they should have worn gowns and gloves and needed to pay better attention to the signage outside the room. The DON and resource nurse manager later stated staff should perform hand hygiene with any glove change and that staff were not implementing correct infection control measures.
