The facility failed to ensure resident records were complete and accurate for two residents reviewed for advance directives and two residents reviewed for admission paperwork. Facility policy required each resident medical record to be maintained complete, accurately documented, readily accessible, and systematically organized. For Resident 6, a care conference review stated the resident had an advance directive and that a copy of the healthcare POA was on file, but the medical record contained no POA paperwork. The Social Services Director stated the reference may have been a typo and that advance directive paperwork was sometimes dropped off at the front desk and not seen by her. For Resident 17, the quarterly assessment showed the resident was cognitively intact, and a care conference summary stated POA paperwork was provided, but no POA documentation was found in the record. For Resident 2, the quarterly assessment showed diagnoses including non-traumatic brain dysfunction, dementia, and encephalopathy, with severe cognitive impairment and inattention; the record contained no admission paperwork, yet a voluntary arbitration agreement was electronically signed by the severely cognitively impaired resident.
