Failure to Perform Hand Hygiene During Meal Tray Delivery

The Valley Health And RehabHamilton, Montana Survey Completed on 01-28-2026

Summary

The facility failed to ensure staff performed hand hygiene while passing meal trays to residents in their rooms. During an observation on 1/26/26 at 7:49 a.m., staff member K delivered a meal tray to a resident, removed the plate cover, returned it to the cart, then took another tray to a different resident's room without sanitizing in between tray deliveries. During an interview shortly afterward, staff member K stated hand sanitizer should be used before passing trays and between each tray, and acknowledged not using it during those trays. Additional observations showed the same issue repeated by other staff. On 1/26/26 at 12:52 p.m., staff member L entered a resident's room with a meal tray, exited, grabbed another tray, and delivered it without sanitizing hands in between. On 1/27/26 at 12:56 p.m., staff member Q passed multiple trays without sanitizing between rooms and later stated staff should sanitize after each tray, but did not know that was the process until the day before. On 1/28/26 at 7:50 a.m., staff member M was observed passing meal trays without sanitizing between resident rooms and stated staff should sanitize every time they leave a resident's room. Staff member H also stated that staff serving residents during meals should perform hand hygiene between trays. Facility documents titled Hand Hygiene and Food Safety Requirements both stated hand hygiene is required between resident contacts and before distributing trays, including washing hands between contact with residents and after collecting soiled plates and food waste.
