The facility failed to store and handle food in accordance with professional standards for food service safety. During an inspection, surveyors observed multiple instances of improper food storage, including an unsealed bag of chicken taquitos and an expired loaf of bread wrapped in plastic wrap left in the freezer beyond its safe use-by date. Additionally, bags of grapes, herbs, and celery in the resident refrigerator were not placed in sealed containers, leaving them exposed to potential cross-contamination and contamination from splash, dust, or other sources. In the dry food pantry, coffee was left open on the counter and sugar was stored in an unsealed bag, both exposed to contamination. Further observations revealed that several dietary aides and the chef did not wear or properly place beard restraints while preparing food, increasing the risk of hair contaminating food intended for resident consumption. Additionally, a cooking pan with a damaged surface was found in use, which could result in pan coating particles contaminating food. Interviews with dietary staff confirmed lapses in following procedures for sealing, labeling, and discarding expired food, as well as inconsistent use of required protective equipment. Record reviews indicated that the facility had policies in place requiring proper food storage, labeling, and the use of hair restraints, as well as the disposal of damaged food contact surfaces. Despite these policies, staff interviews and observations confirmed that these procedures were not consistently followed, resulting in multiple deficiencies related to food safety and sanitation.
