Summary

The facility failed to submit a required five-day follow-up report for a Facility Reported Incident involving an allegation of sexual abuse. The initial allegation was reported to the appropriate authorities, including Adult Protective Services, the Ombudsman, and the Office of Inspector General, and an internal investigation was conducted. The resident involved did not have capacity and reported the incident as having occurred months prior; during a subsequent interview, she did not recall any inappropriate touching.
