Failure to Provide ADL Grooming and Hygiene Assistance
Summary
The facility failed to provide assistance with activities of daily living related to cleanliness and grooming for residents who were dependent on staff for care. The deficiency involved 3 sampled residents and 1 supplemental resident who needed help with bathing, nail care, shaving, and personal hygiene, but were observed with long fingernails, debris under the nails, and long facial hair. The facility policy stated that residents unable to carry out ADLs were to receive the necessary services to maintain grooming and personal hygiene. Resident 70 had diabetes, muscular dysfunction, and weakness, and the MDS showed no refusal of care or memory impairment. The care plan directed staff to provide substantial assistance with bathing and nail trimming on shower days, and the Kardex scheduled showers every Wednesday and Saturday. However, observations on multiple dates showed nails extending about half an inch past the nail bed with brown debris and dry skin underneath. The bathing record documented several bed baths as completed, but no refusal was documented, and Resident 70 stated they did not receive a bed bath and that when a bath was offered and refused because the room was cold, staff did not return to offer it again. Resident 69’s admission MDS showed dependence on staff for personal hygiene and no rejection of care, yet observations showed long, uneven fingernails that remained untrimmed on repeated checks. Resident 17’s MDS showed moderate one-person assistance was needed for personal hygiene, showers, and toileting, and the care plan identified a need for one-person assistance with personal hygiene and oral care; observations showed the resident lying in bed in a hospital gown with long facial hair and long fingernails with black debris. Resident 52’s MDS showed one-person assistance was needed for personal hygiene, toileting, and showers, and the care plan and physician orders directed weekly nail care; observations showed long fingernails with black debris, and the resident stated they needed staff assistance to clip them. Staff interviews confirmed that dependent residents were expected to receive ADLs including nail care, shaving, and personal hygiene, and that refusals should be documented when care was not provided.
