The facility failed to ensure services provided met professional standards of practice for 5 of 25 residents reviewed. The report states nursing staff did not clarify and/or follow physician orders and did not follow medication parameters, affecting Residents 11, 82, 69, 13, and 7. For Resident 13, an RN applied pain patches to both shoulders even though the MAR directed application to the chest area only. For Resident 11, pain medication was given when the resident’s pain level was documented as zero. For Resident 7, narcotic and non-narcotic pain medications were administered outside ordered pain parameters, and a blood pressure medication was not held when the resident’s systolic BP or pulse was below the ordered threshold. Staff interviews confirmed the orders were not followed or needed clarification.
