Care plans not updated to match residents’ current conditions and interventions
Summary
The facility failed to update and revise care plans for 3 of 25 sampled residents whose records were reviewed. The deficiency involved Resident 91, Resident 2, and Resident 69, whose care plans did not reflect current conditions, interventions, or needs documented in assessments, observations, and staff interviews. Resident 91 was admitted with medically complex conditions including recent abdominal surgery. The admission MDS showed the resident could transfer and walk 10 feet with supervision or touching assistance, did not use a wheelchair, did not use bed rails, and required a pressure-reducing device for the chair. However, the care plan still included mobility bars that staff later confirmed were no longer needed for mobility, did not document the resident’s wheelchair use, and continued to show a pressure ulcer prevention need even though staff stated the resident was no longer at risk for pressure ulcer development. Observations showed the resident using a wheelchair without a cushion or pressure-reducing device and using the mobility bars only to hold a fan. Resident 2 had a history of falls, a fall since admission, limited range of motion in both lower legs, substantial assistance needs for bed-to-chair transfers, and use of a walker and wheelchair. After a fall in the room, staff revised the care plan to include a Call Don’t Fall sign as a visual reminder, but the revised falls care plan did not include that intervention. Observations later showed no fall intervention signs in the resident’s room. Resident 2’s dental assessment also documented broken teeth, but the comprehensive care plan only identified a risk for oral/dental problems and did not reflect the actual broken teeth. Resident 69 had a chronic digestive disorder, and the revised care plan directed staff to provide small frequent meals instead of three large meals, but observations showed the resident receiving regular portion meals and staff confirmed the resident was still being served three regular meals a day.
