Summary

The facility failed to thoroughly investigate Resident 114’s falls and did not ensure new interventions were appropriately developed or timely initiated. Resident 114 was admitted with diagnoses including cerebral infarction, hypertension, muscle weakness, and acquired absence of the right leg above the knee, and was able to make needs known. The resident reported falling in the bathroom while trying to transfer from the toilet to the wheelchair, and the incident reports showed falls on 12/04/2025 and 12/17/2025.
