Summary

The facility failed to provide consistent specialized rehabilitative services as required by the care plan for a resident admitted with a fractured femur and legal blindness, who was moderately cognitively impaired and required extensive assistance with activities of daily living. According to the care plan and therapy orders, the resident was to receive occupational therapy (OT) and physical therapy (PT) five times per week for four weeks.
