Summary

Facility staff failed to provide required Resident's Rights training for one dietary aide, as identified during a review of six employee records. The dietary aide in question was hired on 8/26/25, but no credible evidence was found in the employee records to confirm completion of Resident's Rights training. The Director of Nursing (DON) stated that training on resident rights, abuse, and similar topics was expected for all staff, including dietary staff, prior to beginning their duties.
