Summary

The facility failed to ensure that residents could file anonymous grievances, as required by policy. During interviews with five residents at a Resident Council meeting, all stated they did not know how to file an anonymous grievance, and one resident expressed concern about not knowing who to approach with staff-related problems without fear of reprisal. This resident was noted to have no cognitive impairment, as indicated by a BIMS score of 15. Observations on the second floor revealed that blank grievance forms were not accessible to residents, as they were kept behind the nurses' station. Review of the facility's grievance policy showed no mention of anonymous grievance filing.
