Summary

The facility failed to provide required transfer notices to representatives of the Office of the Long-Term Care Ombudsman Division for four months, from July through October 2025. According to the facility's own policy, when an emergency transfer or discharge to a hospital or related institution occurs, several steps must be followed, including notifying the resident's attending physician, the receiving facility, the resident's representative or family member, and preparing necessary documentation. However, during staff interviews and policy review, it was confirmed that the facility did not send transfer notices to the Ombudsman Division representatives during the specified period, as required by regulation 28 Pa.
