Summary

The facility failed to follow CDC recommendations for baseline tuberculosis (TB) screening and testing for three of five newly hired employees. Specifically, the personnel files for these employees showed that only a single-step tuberculin skin test (PPD) was performed, with no documented evidence of the required second step of the two-step testing process. The dates of the initial PPD tests and their readings were recorded, but there was no documentation of a follow-up test for any of the three employees. During an interview, the Director of Nursing confirmed that the second PPD test was not performed for these staff members.
