Summary
The facility failed to implement physician orders for two residents with significant cardiac conditions. One resident with congestive heart failure and hypertension had a physician order for daily weights, but there was no documented evidence that staff weighed the resident on multiple specified dates. Another resident with heart failure and atrial fibrillation had a physician order for daily administration of metoprolol succinate, with instructions to withhold the medication if the heart rate was below 55 beats per minute.
